One hour weekly in home visit/wellness check (1.5 hours for 2 people)
B/P, Temperature, Heart Rate, Oxygen level
Heart, lung, circulation, mentation assessments
Check the home environment for safety
Review any medical reports, upcoming visits and organize them
Medication education, pour medications and make a schedule if needed, and review
Review any medical tests
Education on health issues
Schedule appointments if needed
Communicate with medical team if needed
Weekly updates to family as needed
Availability to call triage phone number during business hours
Additional visits if requested by patient (**additional fee)
